Who needs this form?
Landlords whose tenant signed an N11 or received an N12/N13 but didn't vacate
When to use it
After the agreed or noticed termination date has passed and the tenant remains in the unit.
What happens next
The LTB can order eviction based on the prior agreement or notice. Filing fee: $186 (online) paid to the LTB.
Note: The LTB charges a separate government filing fee ($186 online / $201 by mail), paid directly to the LTB when you file.
